It ensures all agents are assessed on the same criteria, eliminating personal bias and focusing on measurable competencies such as communication, adherence to compliance standards, and problem-solving skills. A questionnaire also acts as a benchmark for tracking progress and identifying areas that require improvement.

Steps to Create an Evaluation Form

  1. Set Objectives

Start by defining the goals of the evaluation. Identify key performance indicators (KPIs) such as clarity of communication, adherence to compliance standards, or problem-solving efficiency. These objectives guide the structure and content of the form, ensuring it captures the most critical aspects of agent performance.

  1. Organize Sections and Questions

Divide the form into logical sections that address different performance areas. For instance:

  • Communication: Assess clarity, tone, and customer engagement.

  • Compliance: Evaluate adherence to policies and regulatory guidelines.

  • Problem-Solving: Measure the ability to resolve customer concerns effectively.

Within each section, include targeted questions and use appropriate formats such as dropdowns, ratings, or multiple-choice questions. Structuring questions this way makes the form user-friendly and facilitates accurate evaluations.\

  1. Assign Weights

Assign relative importance to sections and individual questions. For example, compliance-related questions might have a higher weight if adherence to regulations is a top priority. Weighting ensures that critical competencies significantly influence the overall evaluation score, emphasizing the organization’s priorities.

  1. Ensure Validation and Completeness

Validation plays a critical role in maintaining the accuracy and usability of the evaluation form. The form ensures that each section and question is properly weighted to meet a total of 100%. If the cumulative weights of sections or questions within a section do not equal 100%, the form triggers an error, notifying the user to adjust weights for completeness and balance.
